The official Verdansk Tac Atlas describes underground railway sections, including tunnels north of Hospital and in the southern coastal crags. It also identifies the moving train as a hazard.
These are documented map features, not hidden wall breaches. When this version of Verdansk is in rotation, scout entrances and exits before committing.
